Adams Suite 2000<br/>Phoenix, AZ 85007</p><h3>Temporary Certificate</h3><p>Temporary AP Certificate is available for:</p><ol><li>Endorsement applicants who have met all of the above requirements and have been issued a temporary Arizona RN license or hold a current professional license in good standing in another compact party state.</li><li>New graduate AP applicants who have met all of the above requirements and are awaiting national certification. Applicant must:<ul><li>Request certifying agency to send verification that you have applied for and are eligible to take or have taken an advanced practice certifying examination in their category or specialty area of practice. Verification must come directly from the certifying agency, directly to AZBN.</li><li>Provide written authorization to the certifying body to release the certifying examination results to the Board*</li></ul></li></ol><p>*See temporary certification application.</p><p> </p><p>A form to request a temporary certificate is included with the application. In addition to the completed application and requirements as stated above for permanent certification, please note the following:</p><ul><li>Must submit the required fee.</li><li>Applicants are ineligible for a temporary certificate if they answer “yes” to the application questions of the application. Issuance of a permanent certificate will also be delayed.</li><li>A temporary advanced practice certificate will not be issued until official transcripts, and official letter from your school and verification of eligibility to test for national certification from the testing agency are received by AZBN. The temporary certificate will be mailed to the address on your application. A temporary certificate can be held at the Board office for you to pick up, if you submit a written request with your application.</li><li>The temporary certificate expires in 6 months from date of issue or automatically upon failure of the national certifying exam. Temporary certificates may be renewed for good cause at the direction of the Executive Director. Temporary certificates do expire due to failure of national certifying exam.</li><li>If you do receive a temporary certificate and have not received a permanent certificate at least 10 days before the temporary certificate is due to expire, call Naira Kutnerian at 602-771-7834.</li></ul><h3>Prescribing and Dispensing Authority</h3><p>Nurse Practitioners and Certified Nurse Midwives seeking prescribing and dispensing privileges must complete question 20 prescribing and dispensing AND shall submit written evidence of 45 contact hours of education in pharmacology or clinical management of drug therapy within the three year period immediately preceding this application. You may not prescribe or dispense medications until you receive official notification that prescribing and dispensing authority has been granted. An additional fee for Prescribing and Dispensing is required, please see fees on application.</p><ul><li>Nurse Practitioners or Certified Nurse Midwives must hold current licensure as an RN in Arizona or a compact license with multi state privilege OR</li><li>Be certified by the Board of Nursing within one of the specialty areas identified in the rules of the Board of Nursing at R4-19-501.</li></ul><h4>DEA Registration Number</h4><p>If you are going to prescribe and/or dispense controlled substances, you will need to apply to the Drug Enforcement Administration for a DEA number.</p><ol><li>A DEA number cannot be obtained until you have received prescribing and/or dispensing authority from the Board.</li><li>You must contact the DEA at <a href="http://www.dea.gov">www.dea.gov</a> to obtain an application.</li><li>When the DEA number has been obtained you must notify the Board of your number by sending a copy of the Registration Number.</li></ol><h4>Controlled Substance Prescription Monitoring Program - CSPMP</h4><p>Pursuant to R4-19-512B in the Nurse Practice Act</p><p>An RNP with P & D authority who wishes to prescribe a controlled substance shall obtain a DEA registration number before prescribing a controlled substance. The RNP shall file the DEA registration number with the Board.</p><p>When DEA has been obtained you must notify the Board by completing and mailing the Controlled Substance Prescription Monitoring Program CSPMP form to the Board of Nursing for each DEA number you obtain.</p><p><a href="https://pharmacypmp.az.gov/frequently-asked-questions-faqs">Arizona State Board of Pharmacy FAQs</a></p><h4>Important Notice</h4><p>The <strong>Controlled Substance Prescription Monitoring Program CSPMP</strong> is a statutory program developed to promote the public health and welfare by detecting diversion, abuse, and misuse of prescription medications classified as controlled substance under the Arizona Uniform Controlled Substances Act.</p><p>Arizona Revised Statues § 36-2606, effective December 31, 2015, requires every Arizona medical practitioner, including <strong>Advanced Practice Registered Nurses</strong> who intend to obtain a Drug Enforcement Administration number or who hold one or more DEA registration numbers to also hold a Controlled Substance Prescription Monitoring Program registration issued by the Arizona State Board of Pharmacy. There is NO fee to the practitioner for this registration.</p><p><strong>Helpful Hints on Prescribing and Dispensing Authority</strong></p><p>Only Nurse Practitioners/Certified Nurse Midwives who have been certified by the Arizona State Board of Nursing and meet the other established criteria in R4-19-511 can be granted prescribing/dispensing authority.</p><p>The following information is about PRESCRIBING and DISPENSING:</p><ol><li>A DEA number or license is necessary only if you are going to prescribe controlled substances. To obtain this license, the nurse practitioner must apply to the DEA and pay the required fee. Once this license has been obtained, it must be renewed with the DEA. Controlled substances, Class 2, 3, 4 and 5 can be prescribed. Class 2 cannot be refilled. Class 3 and 4 can be refilled for a maximum of 5 refills in six months. Class 5 can be refilled up to a year.</li><li>Arizona’s prescriptive authority can only be used in Arizona. It cannot be transferred to another state. A CMA has completed a medication training program, and passed an Arizona written and skills test for CMA’s.</p></div>, <div class="accordion-content cc_cursor"><p>CNA certification by Endorsement means that you have graduated from an approved nursing assistant program, taken the state nursing assistant exam, and have been granted CNA certification by another state. If you wish to apply for CNA certification in Arizona, you will need to complete the CNA Endorsement application.</p><p>CNA certification by Examination means that you have graduated from an approved nursing assistant program, want to take the Nursing assistant state exam and obtain an original CNA certificate in Arizona.</p><p><a href="https://www.youtube.com/watch?v=4mh3JQ0dr0U&feature=emb_logo&ab_channel=ArizonaStateBoardofNursing">CNA Certification Video</a></p><p>After more than 20 years regulating Certified Nursing Assistants (CNAs), the Arizona State Board of Nursing (Board) is converting to two levels of nursing assistant:</p><ul><li><strong>Certified Nursing Assistant (CNA):</strong> a person who passes an approved CNA course and the state manual skills/written exams and whose name is placed on the CNA registry. Any substantiated allegations of abuse, neglect or misappropriation of property/theft will be reported to the Office of Inspector General (OIG) and will be listed on the registry.</li><li><strong>Licensed Nursing Assistant (LNA):</strong> a person who meets CNA educational and exam requirements and, in addition, submits an application, fingerprints and fees to the Board. The LNA is regulated by the Board which has broader jurisdiction for any act of unprofessional conduct.</li></ul><h3>Rationale for CNA and LNA Change</h3><p>After more than 20 years regulating Certified Nursing Assistants (CNA), the Arizona State Board of Nursing (Board) is moving to two levels of nursing assistant: CNA and LNA. Current regulation of CNAs, which includes criminal background checks and full Board jurisdiction for all unprofessional conduct, cannot be maintained without either charging the CNA, which is prohibited by federal law, or using the fees paid by RNs and LPNs for their licenses. In 2014 the legislature required the Board of Nursing to propose a self-sustaining model for regulation of nursing assistants. The Board’s model, which was passed into law in 2014, calls for two levels of nursing assistant (CNA and LNA). The requirements for LNA are identical to the current requirements for CNA with one additional requirement: the payment of fees for licensure ($50.00, initial fingerprinting $50.00 and renewal $50). The requirements for the "new" CNA are the minimum required by federal law to be listed on a Nurse Aide Registry and, consistent with federal law, do not include fees for initial placement or renewal on the Registry.</p><h3>NA Training Programs</h3><p>All students completing a nursing assistant program after July 1, 2016 will need to submit proof of legal presence in the United States to D & S (Headmaster) prior to taking the state exams. To facilitate the processing of applications, the Board is requiring all NA training programs to collect the document, scan the document and upload to Headmaster. Ideally, this identification document should be the same as the ID document(s) the student uses for state testing (e.g. AZ driver's license or passport). The name on the document should exactly match the name entered in the system. A comprehensive list of documents acceptable for proof of legal presence is available <a href="/licenses-and-certifications/citizenship-and-alien-status">here</a>.</p><h3>Facilities</h3><p>The titles CNA and LNA are mutually exclusive. All CNAs who are converted to LNA should use the initials “LNA” when identifying themselves. Registry CNAs should use the initials "CNA" when identifying themselves. Your application will be reviewed to determine if you meet the criteria for issuance of a multistate license.
